About the Role
We are seeking a highly skilled Senior Database Engineer to join our engineering team in building and supporting enterprise healthcare platforms.
The ideal candidate will have strong expertise in PostgreSQL database administration, schema design, healthcare data modelling, HL7/FHIR interoperability, cloud and on-premises deployments, data migration, and database performance optimisation.
The role requires close collaboration with application architects, backend developers, DevOps engineers, AI teams, and healthcare integration specialists to design secure, scalable, and high-performance database solutions.
Key Responsibilities
Database Architecture & Design
• Design, develop, and maintain enterprise-grade PostgreSQL databases.
• Create scalable and normalised database schemas for multi-module healthcare applications.
• Design ER diagrams, relational models, constraints, indexes, views, stored procedures, functions, and triggers.
• Review database architecture and recommend improvements for scalability and maintainability.
• Maintain database documentation and data dictionaries.
Healthcare Data & Interoperability
• Design database structures supporting HL7 v2.x messaging and FHIR R4 resources.
• Support healthcare interoperability across HIS, EMR, EHR, LIS, RIS, PACS, and third-party healthcare systems.
• Develop database models for FHIR resources, including:
- Patient
- Practitioner
- Organization
- Encounter
- Appointment
- Observation
- Condition
- Procedure
- Medication
- CarePlan
- Device
- ServiceRequest
- DiagnosticReport
- Questionnaire
- QuestionnaireResponse
- Other FHIR resources as required
- • Work with healthcare integration engines such as Mirth Connect to process HL7 messages and FHIR APIs.
- • Support healthcare master data management and clinical data standardisation.
Database Administration
• Install, configure, upgrade, and maintain PostgreSQL databases.
• Monitor database health, availability, and performance.
• Configure replication, backup, recovery, and disaster recovery strategies.
• Manage users, roles, permissions, and security policies.
• Perform database patching and production maintenance activities.
• Ensure database availability for mission-critical healthcare applications.
Database Performance Optimisation
• Analyse slow-running SQL queries and optimise execution plans.
• Design indexing strategies and table partitioning.
• Tune PostgreSQL configuration for high-concurrency environments.
• Optimise transaction handling and large-volume data processing.
• Monitor database performance using enterprise monitoring tools.
Data Migration & Data Management
• Plan and execute large-scale healthcare data migration projects.
• Develop migration scripts from legacy systems to PostgreSQL.
• Perform ETL activities for healthcare datasets.
• Create and maintain module-wise master data and seed data.
• Design automated data seeding processes for multiple application modules.
• Validate migrated data for accuracy, completeness, and consistency.
• Support production database upgrades and schema versioning.
Cloud & Infrastructure
Deploy and manage PostgreSQL across:
• Google Cloud Platform (Cloud SQL)
• Amazon Web Services (Amazon RDS for PostgreSQL)
• Bare Metal / On-Premises Infrastructure
• Virtual Machine environments
• Docker-based deployments
Work closely with DevOps teams to support:
• Containerised deployments
• Environment provisioning
• High Availability (HA)
• Disaster Recovery (DR)
• Infrastructure migration
• Database monitoring
Security & Compliance
• Implement Role-Based Access Control (RBAC).
• Configure encryption for data at rest and in transit.
• Maintain audit logging and database activity monitoring.
• Support healthcare compliance requirements, including PDPA, HIPAA, and GDPR (where applicable).
• Ensure secure handling of clinical and patient data.
Collaboration
• Work closely with Backend, Frontend, AI/ML, DevOps, QA, and Product teams.
• Participate in solution architecture and technical design discussions.
• Support production deployments and post-release validation.
• Troubleshoot database and integration issues in production environments.
• Provide database best practices and technical guidance to development teams.

• Experience with healthcare platforms, clinical systems, or healthcare interoperability is highly preferred.
• Strong understanding of cloud and on-premises database deployments.
