Role Description Nurse Practitioner/Physician Assistant role is based in the The DFW area, San Antonio, and Houston in Texas and in Atlanta, Georgia and focuses on delivering comprehensive wound care to patients in their residence or care facilities. The clinician will perform wound assessments, develop and implement evidence-based treatment plans, and monitor healing progress in coordination with supervising physicians and multidisciplinary care teams. Daily responsibilities include documenting clinical encounters, updating care plans, educating patients and caregivers on wound management and prevention, and communicating clearly with facility staff and referring providers. The role also involves adhering to established clinical protocols, maintaining regulatory and infection control standards, and contributing to quality improvement initiatives that enhance patient outcomes and care team satisfaction.
Qualifications
- Valid Nurse Practitioner or Physician Assistant license, with authorization to practice in the relevant jurisdiction; completion of an accredited NP or PA program.
- Demonstrated clinical skills in wound assessment, debridement, dressing selection, infection recognition, and ongoing wound management.
- Experience collaborating with physicians, nurses, therapists, and facility staff to coordinate patient-centered care plans.
- Strong documentation, clinical reasoning, and decision-making abilities, with attention to accuracy and compliance with regulatory standards.
- Effective communication and patient education skills, including the ability to explain treatment plans and self-care strategies in clear, accessible language.
- Commitment to compassionate, non-discriminatory care that respects patient dignity, cultural differences, and individual needs.
- Ability to work independently in mobile care settings, manage schedules efficiently, and adapt to different facility environments.
- Prior experience in wound care, geriatrics, long-term care, or home health is highly preferred; wound care certification is an advantage.
