Role Description The Medical Record Reviewer is responsible for reviewing patient charts and documentation to ensure accuracy, completeness, and compliance with internal guidelines and payer requirements. Day-to-day tasks include evaluating medical records for appropriate coding support, verifying documentation against billing submissions, identifying discrepancies, and escalating issues to the relevant teams. The role involves collaborating with medical billing, coding, and quality assurance teams, preparing summaries or reports based on record reviews, and maintaining confidentiality of health information. This is a full-time, on-site role based in Chennai, requiring consistent adherence to deadlines, organizational policies, and regulatory standards.
Qualifications
- Knowledge of medical records, healthcare documentation standards, and basic medical terminology.
- Understanding of medical billing, coding concepts, and insurance/payer requirements (experience with revenue cycle processes is an advantage).
- Strong analytical, documentation review, and attention-to-detail skills to identify inconsistencies or missing information.
- Proficiency with computer systems, electronic health records, and MS Office tools used for data entry and reporting.
- Effective written and verbal communication skills, with the ability to coordinate with cross-functional teams.
- Ability to maintain confidentiality, follow compliance guidelines (HIPAA or equivalent), and adhere to quality standards.
- Capability to work on-site in Chennai, manage time efficiently, and handle repetitive tasks with accuracy.
- Educational background in health information management, nursing, life sciences, or related fields is preferred; prior experience in medical record review or billing is an added advantage.
