Job Summary:
The role of a Care Manager is a multifaceted position crucial for ensuring the seamless delivery of home health care services. This position encompasses a variety of responsibilities, each aimed at maintaining high standards of patient care and operational efficiency. The Care Manager is responsible for coordinating the placement and training of Home Health Workers (HHWs), managing patient care plans, conducting nursing visits, and fostering strong relationships with both patients and home health workers. The role is guided by Life Circle's Care Quality Framework, which emphasizes safety, effectiveness, compassion, responsiveness, and strong leadership.
Key Responsibilities:
Home Health Worker Placement & Replacement
A key duty of the Care Manager is to oversee the placement and replacement of Home Health Workers. This includes providing on-the-job training at the time of placement or replacement, ensuring that no HHW begins their duties without appropriate training. The Care Manager must also train HPs on specific patient care plans and ensure timely placements in adherence to ON TIME OTP's. Ensuring that Health Workers understand their responsibilities and complete necessary training and supervisions is crucial for maintaining service quality.
Patient Care Plan Preparation & Updating
Developing and updating patient care plans is another essential responsibility. The Care Manager creates these plans for all enrolled patients upon enrolment and ensures they are regularly updated to reflect any changes in patient needs. This process includes conducting initial assessments to understand patient care needs and updating care plans to achieve good outcomes and promote a high quality of life based on the best available evidence.
Assessment and New Patients
The Care Manager conducts timely assessments and enrols clients according to company policies. Pre-placement training for Home Health Workers is also provided to ensure readiness for patient care. This ensures that all patient interactions are safe and effective.
Patient-Client Relationship Management
A vital aspect of the Care Manager's role is managing patient-client relationships. This includes addressing all patient-client issues promptly, preventing service discontinuation due to quality issues, and tracking patient health outcomes. Ensuring that clients know how to raise complaints and managing these complaints effectively is also crucial.
Check-in Calls and Visits
Regular check-ins and visits are mandatory to ensure ongoing patient care. The Care Manager makes the required number of check-in calls and visits each month, with no pending visits, and submits visit reports promptly. This ensures that regular patients receive consistent care, and all visits are documented accurately for supervision and audits.
Home Health Worker Relationship Management
The Care Manager also manages relationships with Home Health Workers, ensuring they respond promptly to calls, track attendance, and coordinate replacements for absences. Providing counselling and support to manage work-related stress is also part of the role, ensuring a supportive work environment.
Home Health Worker Health Issue Handling
In case of health or other emergencies involving Home Health Workers, the Care Manager provides immediate attention and support. This ensures that health workers feel safe and supported in their roles.
Positive Brand Projection
Maintaining a positive brand identity is crucial. The Care Manager achieves this by adhering to Life Circle's dress code, grooming standards, and professional conduct guidelines during client interactions. This includes wearing Life Circle attire, remaining well-groomed, and ensuring punctuality for home visits and appointments.
Payment Collection
Timely payment collection from clients is another responsibility. The Care Manager ensures payments are collected by the due date, follows up on overdue payments, and issues provisional receipts, depositing them promptly into the company's bank account.
Daily Visit Number
The Care Manager ensures that 4 to 5 planned visits are conducted every morning without fail, maintaining a consistent level of patient interaction and care. This helps in delivering continuous and effective care to all clients.
Client Feedback Collection
Encouraging clients to leave positive feedback on the company's Google page and Life Circle app is also part of the Care Manager's role, helping to build and maintain a strong online reputation. Regular surveys and telephone quality checks are conducted to gather client views and improve service quality.
Process Adherence
Adhering to processes related to new placements, replacements, and client complaints is critical. The Care Manager tracks open tasks on the mobile app and completes them without reminders, ensuring systems are in place to reduce the risk of harm and abuse. This also includes maintaining accurate records for audits and ensuring medication safety through proper training and risk assessments.
Safety Management:
Ensure the safety of seniors and health workers from abuse and avoidable harm.
Implement safety checklists for falls prevention, infection control, abuse prevention, medication safety, bed sores prevention, and unplanned hospital admissions.
Maintain accurate Medication Administration Records (MAR) and ensure health workers are trained in medicine administration.
Conduct regular risk assessments and provide guidance for mitigating potential risks.
Oversee attendance to ensure timely arrival of health workers
Enforce the use of protective equipment by staff to prevent cross-infection.
Manage and analyze accidents and incidents to identify trends and themes.
All placement following the Life Circle process as guided by the software. No placement outside the Life Circle process.
Business Development
The Care Manager contributes to business development efforts, helping to meet monthly targets and earn incentives. This is essential for the growth and sustainability of the organization and involves promoting high-quality, person-centred care and support.
In summary, the Care Manager plays a pivotal role in ensuring high-quality home health care services, maintaining robust operational procedures, and fostering positive relationships with both patients and Home Health Workers.
