Role Overview
We are seeking a Healthcare Data Ingestion & Interoperability Engineer to lead the architecture, design, and execution of real-time clinical data ingestion pipelines. In this position, you will build and maintain mission-critical data pathways connecting Electronic Health Record (EHR) systems (e.g., Epic, Cerner), Health Information Exchanges (HIEs), and Patient-Generated Health Data (PGHD) platforms directly into cloud and on-premise analytics engines.
The ideal candidate possesses deep hands-on expertise in parsing, modeling, and routing complex clinical message streams, implementing SMART on FHIR protocols, and enforcing strict HIPAA/PHI compliance controls at the API layer.
Core Technical Skills & Tooling Stack
Pillar 1 : Integration Engines & EHR Ecosystems :
- Interface & Ingestion Engines : Enterprise-level mastery of Striim, Mirth Connect (NextGen Connect), and Redox for real-time message routing and transformation.
- Legacy & Modern Clinical Standards : Production-level parsing and processing of HL7 v2 (ORU, ORM, ADT), C-CDA / CDA clinical documents, and FHIR R4 resources.
- EHR API Platforms : Production deployment experience with Epic App Orchard / Epic on FHIR, Cerner Ignite, or equivalent US EHR vendor integration ecosystems (not limited to sandbox testing).
Pillar 2 : FHIR Architecture, Security & Authentication :
- FHIR Modeling & Infrastructure : Hands-on configuration of Azure Health Data Services (or AWS HealthLake / GCP Healthcare API), including FHIR resource modeling and complex RESTful query optimization.
- Authorization & Authentication Protocols : Expertise in SMART on FHIR authorization flows, specifically OAuth 2.0 and JWT client credential flows with asymmetric keys for secure, system-to-system automated EHR integration.
- HIPAA-Aware API Security : Deep understanding of Protected Health Information (PHI), implementing API-layer access controls, automated data minimization, and immutable audit logging.
Pillar 3 : External Networks & Distributed Systems :
- Patient-Generated Health Data (PGHD) : Ingestion engineering across wearable and health aggregator platforms including Terra API, Validic, Apple HealthKit, and Google Health Connect.
- HIE Interoperability Networks : Technical familiarity and data exchange participation with CommonWell Health Alliance, Carequality, and eHealth Exchange.
- Infrastructure & Query Optimization : Capability to operate under on-premise hardware and local network constraints using distributed query engines to execute performant queries across fragmented clinical datasets.
Key Responsibilities
- Pipeline Development : Architect, test, and maintain robust batch and stream ingestion pipelines handling high-volume HL7 v2 and FHIR messages.
- System Integration : Establish secure, automated server-to-server connections with hospital EHRs and clinical networks using OAuth 2.0 asymmetric authentication.
- Data Quality & Validation : Implement schema evolution checks, automated error handling, deduplication, and real-time data transformation rules.
- Compliance & Privacy Guardrails : Ensure all data ingestion channels strictly comply with HIPAA, SOC 2, and data privacy frameworks with complete auditability.
- Performance Tuning : Resolve network bottlenecks and optimize distributed queries over complex on-premise and hybrid cloud environments.
