About the Role

XstremeMD is seeking an experienced and highly skilled Registered Nurse Case Manager to provide telephonic case management and clinical support. This role is part of a team that delivers 24/7 medical coverage, requiring flexibility in scheduling while striving to maintain strong work-life balance.

This position is ideal for nurses who excel in critical thinking, independent decision-making, and high-level clinical assessment — all without direct patient contact.

Key Responsibilities

  • Perform telephonic triage, assessment, and case management 
  • Coordinate and monitor patient care plans 
  • Serve as liaison between patients, providers, employers, and carriers 
  • Ensure timely documentation in EMR systems 
  • Support high-quality, cost-effective care and return-to-work outcomes 
  • Maintain consistent communication and follow-up on assigned cases 
  • Meet productivity and quality benchmarks
Requirements

 

Minimum Qualifications

  • Active Registered Nurse (RN) license (multi-state/compact preferred) 
  • Minimum 2+ years of clinical nursing experience 
  • Strong critical care assessment skills 
  • Proficiency in: 
    • EMR systems 
    • Microsoft Office Suite 
  • Strong computer literacy and ability to multitask across systems 
  • Excellent communication, organization, and critical thinking skills

 

Preferred Experience

  • Emergency Room (ER) or ICU experience 
  • Case management or workers’ compensation experience 
  • Telephonic nursing experience
