XstremeMD is seeking a detail-oriented and clinically strong LPN Case Manager to join our growing team. This is a remote, telephonic role focused on coordinating care, supporting patients, and improving health outcomes — all without direct patient contact.
A top-performing nurse in this role combines clinical judgment, strong communication skills, and the ability to work independently in a fast-paced environment.
Key Responsibilities
- Conduct telephonic case management and patient follow-up
- Perform clinical assessments and identify patient needs
- Document all interactions accurately in EMR systems
- Collaborate with internal clinical teams to support care plans
- Ensure timely and appropriate case resolution
- Maintain high standards of customer service and patient satisfaction
- Manage caseload efficiently while meeting productivity expectations
