About the role
As Head of Health and Social Care Integration, you will play a key role in developing and overseeing the delivery of a sustainable change programme which will improve the health and wellbeing of Wokingham residents.
This programme of work will contain supporting the delivery of common schemes across the Integrated Care Partnership in Berkshire West and the delivery of local schemes, including the Better Care Fund, to meet local needs and reduce health inequalities in accordance with the council's ambition of becoming a 'Marmot Borough'.
The Wokingham Integrated Partnership (WIP) oversees an extensive locality-wide transformation programme, and the role of Head of Health and Social Care Integration is pivotal in its delivery.
The relationships across the main partners and stakeholders are critical to the success of transformation. This role will take a lead in managing the delivery of programmes and ensuring key relationships are developed and maintained, and in facilitating collaborative working.
You will be responsible to ensuring there are strong links with Adult Social Care operational colleagues, in regards the design and review of integration work streams, and in forging close links and opportunities for joint working with colleagues in the Integrated Care Board.
