About the Role
Cosentus is hiring a Full-Stack AI Engineer with mandatory healthcare Revenue Cycle Management experience.
You will build and improve production-grade AI applications across medical coding, eligibility verification, claim submission, claim-status tracking, denial management, EOB/ERA processing, payer follow-up and revenue-cycle analytics.
Our platforms work across core RCM workflows including CPT/ICD coding, charge entry, X12 837 claim submission, payer adjudication, EOB extraction and denial categorization.
This is not a general-purpose AI role. Candidates must understand how healthcare claims and payer workflows operate.
Key Responsibilities
- Design and develop full-stack applications for healthcare RCM workflows.
- Build AI and LLM-based systems for:
- Medical coding
- Eligibility verification
- Claim-status follow-up
- Denial identification and root-cause analysis
- EOB and ERA extraction
- Payment and underpayment analysis
- Appeals and revenue-recovery workflows
- Process and normalize healthcare data including:
- X12 837 claims
- X12 835 remittances
- ERA and EOB documents
- CPT, ICD-10, CARC and RARC codes
- Build APIs, backend services, databases and responsive user interfaces.
- Integrate with payer portals, clearinghouses, EHRs, practice-management systems and third-party healthcare APIs.
- Develop LLM workflows using prompt engineering, RAG, structured outputs, tool calling and AI agents.
- Build OCR and document-processing pipelines for clinical notes, EOBs and supporting documentation.
- Implement confidence scoring, validation rules, human review and continuous feedback loops. Cosentus workflows use confidence-based automation, structured financial extraction and human review for lower-confidence results.
- Monitor accuracy, latency, reliability and cost of AI workflows.
- Ensure systems are secure, scalable and suitable for handling protected healthcare information.
Mandatory Requirements
- Previous hands-on experience in US healthcare Revenue Cycle Management is mandatory.
- Strong understanding of the end-to-end revenue cycle, including:
- Patient eligibility
- Medical coding
- Charge entry
- Claims submission
- Payer adjudication
- Payment posting
- Denials
- Appeals
- Accounts receivable follow-up
- Experience working with 835, 837, ERA, EOB, CPT, ICD-10, CARC and RARC data.
- Strong full-stack development experience using technologies such as:
- TypeScript and JavaScript
- React and Next.js
- Node.js and/or Python
- FastAPI or similar backend frameworks
- PostgreSQL and SQL
- REST APIs and webhooks
- Practical experience building applications with LLMs, AI agents, RAG or document extraction.
- Experience integrating external APIs and debugging complex production workflows.
- Strong understanding of data validation, exception handling and auditability.
Preferred Qualifications
- Experience with AWS, Bedrock, OpenAI, Anthropic or similar AI platforms.
- Experience with healthcare clearinghouses or payer systems such as Availity, Change Healthcare or Optum.
- Familiarity with medical coding automation, claim-status automation or denial analytics.
- Experience with OCR, voice AI, workflow automation or RPA.
- Understanding of HIPAA, PHI handling, encryption and healthcare data-security requirements.
- Experience building human-in-the-loop AI systems.
