Requirements
- High school graduate or equivalent
- Successfully completion of an Indiana's Certified Nursing Assistant program and license
- Demonstrate skills and ability in working with and understanding the needs of the residents
- Ability to communicate effectively with residents, family members, staff members, volunteers, and outside agencies
- Handle confidential information with professional discretion in accordance with facility, state, and federal regulations
- Desire to work with geriatric population
- Long-term care experience preferred
- Willingness to work in a team atmosphere

Summary
The Transportation CNA is responsible for safely accompanying residents to scheduled medical appointments and community outings while providing compassionate, resident‑focused care. When not engaged in transportation duties, this position provides hands‑on nursing care within the facility, supporting residents’ daily needs in accordance with individualized care plans and facility policies.
Key Responsibilities:
Transportation & Appointment Support
- Escort residents to scheduled medical appointments, treatments, and approved outings outside the facility
- Assist residents with safe transfers in and out of vehicles, wheelchairs, and medical offices
- Remain with residents during appointments as needed to provide support and communicate information back to facility staff
- Ensure residents’ comfort, dignity, and safety during transportation and off‑site visits
- Communicate appointment outcomes, concerns, or changes to nursing staff upon return
Resident Care & Support
- Provide direct resident care when not on transportation assignments, including assistance with activities of daily living (ADLs)
- Assist residents with bathing, dressing, grooming, toileting, mobility, and feeding as assigned
- Observe and report changes in residents’ condition to licensed nursing staff
- Promote resident independence, dignity, and well‑being at all times
- Follow all care plans, safety protocols, and infection control procedures
Documentation & Communication
- Accurately document care provided and observations per facility policy
- Maintain timely communication with nursing staff, supervisors, and administrative team
- Uphold HIPAA and confidentiality standards
Safety & Compliance
- Ensure adherence to safety practices during transportation and resident care
- Use proper body mechanics and transfer techniques
- Participate in training and in‑service education as required
Qualifications:
Required
- Current Certified Nursing Assistant (CNA) certification in good standing
- Ability to work Monday through Friday on a full‑time basis
Skills & Abilities
- Strong interpersonal and communication skills
- Ability to assist residents safely during transfers and mobility activities
- Dependable, organized, and able to manage time effectively
- Ability to interact professionally with residents, families, and healthcare providers
- Physical ability to perform CNA duties, including lifting, standing, and walking
Physical & Work Requirements
- Ability to lift, push, or pull residents with assistance as needed
- Ability to stand, walk, bend, and assist with transfers throughout the shift
- Ability to lift, push and pull at least 50 pounds
- Work performed both inside the facility and in community settings
