Description

As the Home Health Physical Therapist, you will provide physical therapy services in accordance with the plan of care. Provide initial and on-going assessment and evaluation of patient response to therapy. Accurately document evaluation and treatments provided.

MAJOR ACCOUNTABILITIES/ESSENTIAL JOB DUTIES:

  • Provide services in accordance with accepted standards of practice for physical therapy
  • Document all therapy interventions
  • Provide initial and ongoing assessment and evaluation of patient/family response to therapy
  • Develop and revise the plan of care in consultation with the physician and other care-team members
  • Provide therapeutic treatments and evaluation of equipment needs as applicable
  • Advise and consult with the family and other agency personnel as applicable
  • Complete documentation timely for all assessment and evaluation data, treatments, and patient’s response to therapy intervention
  • Evaluate outcomes of care and complete the OASIS assessment accurately
  • Provide discharge planning and evaluation
  • Supervise physical therapy assistants at least every 30 days and home health aides every 14 days when appropriate
  • Participate in in-service programs
  • Comply with agency policies and procedures relative to infection control and safety

REQUIRED KNOWLEDGE, SKILLS AND ABILITY:

  • Current, unrestricted license in the Commonwealth of Virginia as a Licensed Physical Therapist
  • Current CPR certification for healthcare provider
  • Demonstrated ability to read, write, and speak English
  • Must possess good verbal and written communication skills
  • Must be able to meet deadlines, work under limited supervision
  • Valid Driver’s License in the State of Virginia with a minimum age 21 to drive for the organization
  • Motor Vehicle Driving Record satisfactory to the Agency and its insurance providers
  • Proof of motor vehicle insurance

REQUIRED EDUCATION AND EXPERIENCE:

  • Passed an examination for physical therapists approved by the state in which physical therapy services are provided OR graduated after successful completion of a physical therapist education program approved by one of the following:
    1. The Commission on Accreditation in Physical Therapy Education (CAPTE)
    2. Successor organizations of CAPTE
    3. An education program outside the United States determined to be substantially equivalent to physical therapist entry level education in the United States by a credentials evaluation organization approved by the American Physical Therapy Association or an organization identified in 8 CFR 212.15(e) as it relates to physical therapists
