Responsibilities:
- Screen or evaluate patients with ADL physical functioning disorders and develop an appropriate plan of care.
- Utilize standardized assessments and evidence-based practice to support clinical interventions.
- Provide rehabilitative, skilled, and medically necessary treatment intervention to patients with ADL physical functioning disorders.
- Administer various procedures as part of the rehabilitation plan including but not limited to: ADL retraining; therapeutic activities; therapeutic exercises; modalities; use of supportive and assistive devices; etc.
- Assess patients’ progress and adjust treatments accordingly.
- Evaluate treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
- Provides consultation and counseling to patients, as well as families, caregivers, and other service providers related to the physical disorders.
- Effectively communicate with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
- Provide comprehensive discharge summary of services provided.
- Document all regulatory and clinically required information in the electronic documentation system and maintain patient records according to facility policy.
- Accurately record treatment minutes for services provided.
- Participation in required meetings including but not limited to: patient care conferences and facility staff meetings.
- Participate in facility required trainings and in-services.
