<strong>Provide Person-Centered Support:</strong> Engage individuals in meaningful activities based on their preferences, strengths, and goals outlined in their Individual Service Plans (ISPs).</div><ul>

<strong>Facilitate Community Integration: </strong>Accompany and support individuals during community outings, volunteer opportunities, and social engagements to promote independence and social inclusion.</li>
<li>
<strong>Support Daily Activities: </strong>Assist with daily routines such as meal preparation, feeding, toileting, hygiene, and mobility while encouraging maximum independence, community activities, accompanying to doctor's appointments, provide transportation, drive as needed</li>
<li>
<strong>Skill Development:</strong> Support individuals in developing and enhancing skills related to communication, self-care, vocational training, and social interactions.</li>
<li>
<strong>Monitor and Document Progress:</strong> In accordance with regulatory and agency requirements, maintain accurate daily documentation of participation, behaviors, incidents, and progress toward goals in the Therap platform.</li>
<li>
<strong>Ensure Safety and Well-Being:</strong> Implement safety protocols and provide supervision to ensure a safe, supportive, and respectful environment.</li>
<li>
<strong>Implement Behavior Support Plans: </strong>Follow strategies and interventions as outlined in each individual’s behavior support plan to address and manage behavioral needs.</li>
<li>
<strong>Collaborate with Team Members: </strong>Communicate effectively with coworkers, supervisors, clinicians, and families to coordinate support and report concerns or changes in condition.</li>
<li>
<strong>Promote Dignity and Advocacy:</strong> Support each individual's rights, dignity, and choices and advocate for their needs and preferences.</li>
