Our Clinical Team supports individuals through Home and Community-Based Services (HCBS) and Community Oriented Recovery and Empowerment Services (CORE) programs under the Office of Mental Health. Through this work, we’ve helped individuals achieve incredible milestones—from earning degrees to rebuilding relationships and regaining independence.


About the Role

As a Community Mental Health Provider, you will play a hands-on role in supporting individuals on their recovery journey. This is a highly impactful, community-based position focused on building skills, fostering independence, and helping clients navigate real-life challenges in their homes and communities.

You’ll work directly with individuals to strengthen social, emotional, and practical life skills—helping them move forward with confidence and purpose.

Requirements

  What You'll Bring

· Provide HCBS and CORE services in clients’ homes and community settings

· Support individuals in developing coping strategies, adaptive behaviors, and relapse prevention skills

· Assist clients in accessing and maintaining community resources (e.g., SNAP, housing, mental health services)

· Help individuals build social and interpersonal skills, including communication and relationship-building

· Promote independent living skills such as meal planning, budgeting, and daily routines

· Facilitate recovery-oriented activities that support long-term stability and success

· Maintain consistent communication and check-ins with a clinical supervisor for support and development

· Document services and progress in alignment with program and compliance standards

· Perform additional duties as needed to support client success


What Makes You a Great Fit

· High School Diploma or GED required

· At least 1 year of experience in mental health, substance use recovery, or a related human services field

· Knowledge of community resources (DSS/SSI, housing, transportation), SUD supports, and disability accommodations is preferred

· Ability to work independently in community-based settings

· Strong communication, empathy, and relationship-building skills

· A passion for helping individuals achieve independence and recovery
