You will support families during well-child visits and follow-up interactions.<br><br> </div><div>You will focus on:<br><br> </div><ul> <li>Child development</li> <li>Social-emotional functioning</li> <li>Parenting support</li> <li>Early identification of concerns</li> <li>Connecting families to services</li> </ul><div>This role provides short-term, preventive, and consultative support.<br> It does <strong>not</strong> include long-term therapy or ongoing psychotherapy.<br><br> </div><div> <br>What a Typical Day May Include<br><br> </div><div> <strong>Supporting Families<br></strong><br> </div><ul> <li>Promote the HealthySteps program to families and providers</li> <li>Conduct well-child visits (in-person or virtual) focused on development and relational health</li> <li>Provide guidance on child behavior, development, and parenting concerns</li> <li>Build strong, trusting relationships with caregivers</li> </ul><div> <strong>Screening & Early Identification<br></strong><br> </div><ul> <li>Conduct developmental, behavioral, and social-emotional screenings</li> <li>Share results with pediatric providers</li> <li>Identify early signs of delays, behavioral concerns, or family stress</li> </ul><div> <strong>Care Coordination<br></strong><br> </div><ul> <li>Coordinate care with the primary care team</li> <li>Participate in supervision and team meetings</li> <li>Document visits, screenings, and referrals in the EHR</li> <li>Accompany families to care appointments when appropriate</li> </ul><div> <strong>Referrals & Follow-Up<br></strong><br> </div><ul> <li>Connect families to community resources (early intervention, behavioral health, social services)</li> <li>Support families in completing referrals</li> <li>Follow up to ensure services are accessed</li> <li>Use closed-loop referral practices when possible</li> </ul><div> <strong>Parent & Community Engagement<br></strong><br> </div><ul> <li>Facilitate parent support groups</li> <li>Manage community referrals and follow-ups</li> <li>Maintain a family support line and respond promptly</li> </ul><div> <br>Program Participation<br><br> </div><ul> <li>Track caseload and service levels</li> <li>Participate in HealthySteps training and coaching</li> <li>Follow HealthySteps guidelines and evidence-based practices</li> <li>Document services to support program reporting and grant requirements</li> </ul><div> <br>Practice Approach<br><br> </div><div>The HealthySteps Specialist works in a relationship-centered, family-first way.<br><br> </div><div>You will:<br><br> </div><ul> <li>Deliver culturally responsive, trauma-informed care</li> <li>Use a strengths-based approach</li> <li>Respect diverse family backgrounds</li> <li>Reflect on your work and commit to ongoing growth</li> </ul><div> <br>Required Qualifications<br><br> </div><ul> <li>Master’s degree in Social Work, Psychology, or Counseling</li> <li>Licensed in New York State (LMSW, LCSW, LMHC, LPC, or equivalent)</li> <li>Experience working with children ages 0–3 and caregivers</li> <li>Strong communication and engagement skills</li> <li>Ability to work as part of a clinical team</li> <li>Comfort discussing sensitive topics with families</li> <li>Bilingual Proficiency in English and Spanish is required</li> </ul><div> <strong> <br></strong><br> </div><div> <br>Preferred Qualifications<br><br> </div><ul> <li>Experience using developmental screening tools</li> <li>Familiarity with pediatric primary care or integrated behavioral health</li> <li>Experience in grant-funded or evidence-based programs</li> </ul><div> </div> </div>
