Memory Care Director (LPN) – Assisted Living Community An established assisted living community is seeking a compassionate and experienced Licensed Practical Nurse (LPN) to serve as our Memory Care Director and help lead and elevate our growing memory care program. This is a hands-on clinical leadership role for an LPN who thrives in the day-to-day—supporting residents, guiding team members, and ensuring high-quality, person-centered care for individuals living with dementia and cognitive impairment. The ideal candidate combines strong nursing skills with leadership ability and a genuine passion for memory care. Key Responsibilities: Provide clinical oversight and coordination of care for residents in the memory care neighborhood Administer medications and treatments in accordance with physician orders and state regulations Monitor resident health status, recognize changes in condition, and communicate appropriately with providers and families Partner with the Health & Wellness Director to ensure compliance with all clinical policies, procedures, and regulatory requirements Supervise and support care staff (CNAs/Med Techs), including training, coaching, and performance oversight Maintain a strong daily presence on the floor, assisting with resident care and engaging with families and team members Assist in care planning, assessments, and service plan updates to ensure individualized, resident-centered care Promote safe, structured routines that support residents with cognitive impairment Support behavior management and redirection techniques using a positive, person-centered approach Ensure proper documentation of care, incidents, and changes in condition Qualifications: Current, active LPN license in the state of Florida Experience in memory care, dementia care, or assisted living preferred Strong clinical assessment and medication management skills Knowledge of dementia progression, behaviors, and best practices for care and redirection Proven ability to lead, train, and support caregiving team members Compassionate, patient, and relationship-driven approach to care Strong communication skills with residents, families, physicians, and team members Ability to remain calm, organized, and effective in a fast-paced environment Hands-on leader who leads by example and maintains high standards of care Opportunity: This role offers the opportunity to make a meaningful impact by combining nursing expertise with leadership—helping shape the clinical quality, team culture, and overall resident experience within a dedicated memory care program.
