​The Role

MediSync AI is seeking a Tech-Savvy Registered Nurse to deliver fully managed connected care. You will provide virtual support, conduct assessments, and manage patient outcomes using our innovative AI platform. This role is central to our Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) initiatives.

​What You’ll Do

​Conduct virtual patient assessments and triage.
​Coordinate care plans for patients with chronic conditions (CCM).
​Monitor real-time physiologic data and intervene when necessary (RPM).
​Educate patients on treatment plans and digital health tools.
​Collaborate with US-based physicians and healthcare teams.

​What We’re Looking For

​License: Valid RN license (or equivalent).
​Experience: Proficiency in Telehealth, EHR systems, and digital communication.
​Expertise: Strong clinical skills in chronic disease management.
​Soft Skills: Exceptional active listening and "webside manner."
​Reliability: Ability to manage a remote workload during US business hours.
