Registered Nurse - Home Health - Per Diem

Under the general supervision of the Director of Nursing or designee, the Registered Nurse (RN) Home Health Per Diem provides skilled nursing care to patients/clients in their home and community settings. The RN coordinates and oversees the provision of care by other members of the home health care team, and serves as a resource and mentor to these team members. The RN plans, implements, evaluates, and documents the care provided to patients, in accordance with the patient's plan of care and agency policies and procedures.
