Job Summary
The Healthy Steps Specialist is responsible for the coordination and implementation of projects and tasks associated with Summit Healthcare’s Healthy Steps for Young Children Program service delivery. This position is responsible for completing projects and tasks that are cross-functional and complex in nature, supporting many tasks concurrently, motivating personnel, managing competing demands, reacting well under pressure, overcoming obstacles, and leading and influencing others.
Essential Functions
- Conducts team-based well child visits with parents and their children alongside the primary care provider.
- Conducts consultations on a short-term basis for needs regarding development and/or behavioral concerns such as sleep, positive parenting, picky eating, etc.
- Participates in reflective supervision meetings and regular team meetings.
- Provides referrals and tracks follow up to help connect families to community resources.
- Engages in continuous quality improvement initiatives to ensure the Healthy Steps model is delivered with fidelity.
- Documents all clinical activity and care coordination in patient record.
- Maintains open communication with members of the care team.
- Participates in professional development activities.
- Represents Summit’s community outreach services and programs within environments promoting the importance of early childhood health and development.
- Supports families with young children in regard to information about child development and health, care coordination, and parent advocacy.
- Participates in special projects as directed.
Other Duties
- May facilitate parent/caregiver support groups or play groups for children.
- Participates in departmental and association wide informational meetings and inservices, including staff meetings, association wide forums, and seminars.
- Reviews department and association wide policies and procedures annually. Develops and maintains new policies and procedures as needed.

Abilities
- This position requires proficiency in general office equipment (including fax, printer, copy machine and phone systems) and computer programs, such as internet, e-mail, Microsoft Office, Kronos, and the Health Information System.
- The Healthy Steps Specialist must be service oriented and have excellent decision-making, judgement, interpersonal, public speaking, verbal and written communication, presentation, investigative, analytical, customer service, organizational, multi-tasking, professional interpersonal and time management skills, and the ability to prioritize work.
- Knowledge of and experience with early childhood growth and development, parent-child relationships, infant mental health, and/or family systems.
- Supports families with young children in regards to information about child development and health, care coordination, and parent advocacy during Well Child visits.
- Must read, write, speak, and understand English.

Physical Demands
Exerts up to 10 lbs. of force occasionally and/or a negligible amount of force frequently or constantly to lift, carry, push, pull, or otherwise move objects, including the human body. Involves sitting most of the time, but may involve walking or standing for brief periods of time. The worker is subject to substantial overtime, evening meetings, on-call status, contact with chemicals, contact with infectious fluids, heavy lifting, constant standing, extensive close work, extensive computer work, and encounters with upset/disturbed individuals.
Required Education and Experience
- BLS Certification (within 30 days of hire).
- Bachelor’s degree in one of the following areas: Child Development, Psychology, Social Work, Counseling, Early Childhood, Nursing or related field.
Preferred Education and Experience
- Master’s degree.
