About the role

JOB SUMMARY:

The Referral Coordinator will be integrated into the healthcare team to promote patient centered care, have frequent contact with providers and the healthcare team, and support the closed-loop referral process.

 

 

MAJOR ACCOUNTABILITIES / CRITICAL RESPONSIBILITIES:

· Maintains ongoing tracking and appropriate documentation of referrals in the EMR

· Ensures complete and accurate registration, including patient demographic and current insurance information.

· Assembles information concerning patient's clinical background and referral needs.

· Where necessary, conducts pre-authorizations for referrals.

· Reviews details and expectations regarding the referral with patients.

· Schedules referral visits in a timely manner and informs patient of scheduled appointments

· Ensures that referrals are addressed in a timely manner and conducts appropriate follow-up on all open referrals.

· Demonstrates ongoing commitment to customer service excellence and supports the customer service plan. · Completes required continuous training and education, including department specific requirements.

· Complies with departmental organizational policies and procedures and adheres to external agency requirements.

· Performs other duties as assigned.

 

QUALIFICATIONS/REQUIREMENTS:

 

· High School Diploma or equivalent required with a minimum of two years of related medical office experience preferred

· Demonstrated strong interpersonal, organizational skills.

· Effective communication skills, both verbal and written English proficiency required.

· Ability to function independently with minimal supervision within the scope of knowledge and authority of the position.

· Solid phone/customer service skills.

· Knowledge of office equipment operation, (EMR, telephone system, fax machine, copy machine, etc.).

· Ability to handle sensitive and confidential information.

· Careful attention to detail.

· Ability to work both in a team and independently.

· Ability to demonstrate professionalism.

 

PATIENT POPULATION SERVED:

All practice patients as needed

 

MANUAL SKILLS:

Significant portions of daily assignments involve application of manual skills requiring motor coordination in combination with finger dexterity, (i.e., typing on computer keyboard, use of telephone).

 

PHYSICAL EFFORT:

Duties involve some exertion of physical effort: ability to lift 5-19 lbs required.

 

PHYSICAL ENVIRONMENT:

Generally pleasant working conditions in a normal office environment. Nature of duties performed present little or no potential for job-related accidental injury.
