Job details:
Provide social and emotional support to residents and understand their individual needs.
Conduct regular interactions with residents and monitor their social and emotional well-being.
Assist in developing individual care and rehabilitation plans in coordination with the care team.
Coordinate with family members and maintain regular communication regarding residents.
Support residents in maintaining family and community relationships.
Conduct counselling and supportive sessions as required.
Maintain resident case files, assessments, progress notes, and other documentation.
Coordinate with doctors, therapists, caregivers, and other professionals for comprehensive resident care.
Support residents in accessing government schemes, benefits, and other available services.
Organise recreational, social, and community-based activities for residents.
Identify social, behavioural, or family-related concerns and report them to the Manager.
Assist the Manager in resolving resident and family concerns.
Participate in awareness programmes, outreach activities, and community engagement initiatives.
Maintain confidentiality and ensure residents are treated with dignity, respect, and compassion.
