About the company

Kindred Transitional Care and Rehabilitation - Silas Creek is located in the multi-cultural city of Winston Salem, across the street from Forsyth Medical Center. We offer short-term rehabilitative as well as long-term care - a full range of medical, nursing, rehabilitation and social services to treat and support each of our patients and residents. Many of our patients go home after a few weeks and, for those who are unable to return home right away, we provide safe, compassionate care in an environment that fosters independence and dignity. One of our most popular attributes is our Angel Care Program. This program is designed to provide each patient and resident with a dedicated staff member, who makes regular visits to check on the well-being of your or your loved one. Each individual's 'angel' will communicate with family members on a monthly basis, and is available to speak with family members, about their loved one's care.
