Family Forward supports families to have healthy pregnancies and successful early childhood development through resource linkage, education, and community-building. Family Forward meet with clients bi-weekly in their homes to provide intensive case management and connect families to needed resources promoting health, wellness, safety, and stability.
Job responsibilities include:
- Attend community meetings and events as assigned by the Director and Deputy Director.
- Meet with clients bi-weekly in their homes to provide intensive case management and connect families to needed resources, promoting health, wellness, safety, and stability
- Carry and maintain a caseload of 25 families, providing intensive case management and resource linkage. Maintaining appropriate case notes, conducting necessary assessments, and entering data into the system in order to track program progress & outcomes.
- Participate in outreach events to recruit new families.
- Participate in parent workshops and other community building events.
- Participate in and assist with preparing for quarterly family events.
- Presenting the program to partners, funders and other professionals within the community.
This is a grant funded position that work with inner city families in the Chattanooga area.
Occasional evening or weekend work may be required.
Qualifications:
- Must have a Bachelor’s degree in Social Work, Psychology, Child & Family Studies or a related field.
- Bilingual English and Spanish is preferred
- Experience working with inner cities families in a case management model.
- Must have a valid Driver's License and reliable transportation with valid insurance per TN state requirements in order to travel to client homes and community partner meetings within the city.
- Must be organized and able to manage case management notes, assessments, and keep up with data entry requirements.
- Must be professional in demeanor and able to represent the agency well in the community.
- Must be a team player and able to work cohesively in a team environment.
