Summary
The focus of this role is to develop relationships to support care and quality of life. Through these relationships, the Service Coordinator plays an important role in setting the tone for those who work and live at Shalom Park. The Unit Secretary / Service Coordinator assists nursing leadership with administrative and clinical functions on the floor and assisting CNAs as needed by performing the following duties.
Essential Duties and Responsibilities
- Works closely with neighborhood care partners to ensure nursing resources, clinical supplies, and equipment are available and organized to support relationship-based care
- Coordinates scheduling patterns with nursing leadership to ensure that neighborhoods are staffed to establish and maintain relationships
- Coordinates and communicates appointment and transportation scheduling in addition to assisting with scheduling of care conferences and family meetings
- Contributes to Shalom Park community culture by demonstrating safety awareness while nurturing relationships
- Organizes administrative functions such as transcribing orders and supports clerical and record keeping including the RAI process, incident management, Care Tracker, medical records, etc.
- Coordinates admissions and discharges, including equipment and order management to ensure a smooth transition
- Supports service during meals.
- Receives incoming calls, visitors, and deliveries and conveys information clearly to callers, visitors, residents and staff
- Maintains a friendly and warm demeanor at all times; retains composure and objectivity under pressure
- Other duties may be assigned
EDUCATION and/or EXPERIENCE
Certified Nursing Aide (CNA) certification and related long-term care administrative and clinical experience
CERTIFICATES, LICENSES, REGISTRATIONS
Current unencumbered Colorado Certified Nurse Aide (CNA) certificate
