The Neighborhood Networks Program Supervisor provides overall programmatic coordination, and supervision of the Neighborhood Networks (NN) team. Through direct oversight and supervision, NN Program Supervisor ensures compliance with project standards and objectives. The position may be eligible for remote work not to exceed maximum 3 days per week.
ESSENTIAL JOB FUNCTIONS AND RESPONSIBILITIES:
- Supervises and directs program staff consisting of 6-8 Enhanced Care Management (ECM) and Housing Navigator (HN) Lead Care Managers (LCM)
- Conducts regular supervision meetings with staff; escalates major concerns to director.
- Responsible for the onboarding and training new staff; provides staff training and development insights and recommendations to manager/director
- Provides budget allocation input; at direction of manager/director, tracks specific budgetary elements such as office, program, or client expenses to ensure spending aligns with budget.
- Participates in hiring process with manager/director to include prescreening, co-interviewing, and evaluation of potential candidates, and contributes to staff annual performance evaluations.
- Manage client caseload during staff vacancies, that may include outreach, home visits, client interviews, enrollment, timely and accurate recording of case notes, and coordinate specific intensive client services.
- Advocate on behalf of clients with health care professionals, work with medical facility/hospital
- staff to ensure smooth and well supported transitions in care, guide clients to maintain Medi-Cal
- eligibility, and other financial resources as appropriate.
- Conducts outreach, community engagement, and supports capacity building goals to develop new and maintain existing stakeholders and strategic partners.
- Plan and coordinate meetings to include location, participant materials, and program manuals.
- Serves program point of contact and demonstrates programmatic expertise by providing technical assistance to both staff and clients.
- Participate in all mandatory SAY, funder, and partner meetings
QUALIFICATIONS (Education, Experience and Certifications):
- Bachelor’s degree in social work, public health, psychology, Education, or related field preferred
- Minimum of 3 years job related experience
- Lived or Allyship experience working with complex physical, behavioral, or developmental health needs, individuals at risk of or experiencing homelessness, and/or individuals transitioning from incarceration.
- Demonstrates solid job knowledge for scope of work assigned.
- Competency and experience in working with diverse cultures.
- Access to reliable transportation and willing to use it for job-related tasks.
