River Valley Family Health Centers is looking for a licensed Behavioral Health Provider to join our team in Montrose, CO. This person provides integrated behavioral health services within a community-based primary care clinic, delivering culturally responsive, evidence-informed assessment, brief intervention, crisis response, and psychotherapy for individuals and families across the lifespan. Collaborates closely with medical providers and the interdisciplinary care team to support whole-person care, including chronic disease and substance use treatment (e.g., MAT), and connects patients to appropriate community resources. Maintains timely, accurate documentation, care plans, and billing support in the electronic health record in accordance with clinic policy, ethical standards, and HIPAA requirements.

A day in the life of a Behavioral Health Provider includes:
- Performing intakes, assessments, and developing treatment plans for patients.
- Providing family treatment, crisis intervention, short-term, and traditional outpatient behavioral health therapy within the clinic using various therapy models.
- Monitoring patient progress and adherence to treatment plan and documentation in accordance with related policies in the EMR.
- Work with an interdisciplinary team to provide behavioral health services to patients receiving Medication Assisted Treatment (MAT) for substance use disorders, and coordinating Behavioral Health services with the medical team.

Qualifications
- Licensed LCSW/LPC or SEC/LSW/LPCC by the State of Colorado
- LAC or CAS preferred
- BLS/CPR certification required
- Ability to produce written documentation of clinical work in a manner readily understood by other providers.
- Ability to demonstrate appropriate clinical judgment when assessing the level of risk and determining resolution of client emergencies, resulting in a safe outcome.
- Experience providing behavioral health services in a community=based setting serving at-risk and or underserved populations.
- Knowledge of evidence-based treatment models for opioid and alcohol use disorders, including Medication Assisted Treatment.
- Verbal and Written Communication Skills.
- Working knowledge of computers, including Electronic Medical Records, Microsoft Word, Excel, Outlook, scheduling, and timekeeping software.
- Must demonstrate cultural sensitivity when working with individuals and groups of diverse racial, ethnic, economic, and social backgrounds.
- Ability to establish and maintain effective, courteous working relationships with patients, their family members, staff team members, and the community.
- Ability to organize and prioritize workload in a sometimes-hectic environment with frequent interruptions.
- Ability to travel between clinics as needed; may attend meetings and conferences outside the service area.
- Ability to work flexible hours to meet job requirements.
- Valid Driver License required.
This position requires long periods of standing and walking, sitting at a computer, the ability to lift, carry, push, and pull up to 35 pounds, etc. Employees are exposed to blood, body fluids, and other occupational hazards, and are required to complete mandatory vaccines, such ad Tdap, Hep A, Hep B, MMR, Varicella, and the yearly flu vaccine.
