Clinical Care Navigator

As a Clinical Care Navigator at Rightway, you will be responsible for guiding members through the healthcare system to help them access the care and services they need. This role requires strong communication and customer service skills, as you will be the main point of contact for Rightway members. Responsibilities include: - Engaging with members to understand their healthcare needs and guide them to appropriate services - Educating members on their benefits, plan information, and available resources - Connecting members with in-network providers and scheduling appointments - Helping members navigate the healthcare system and resolve any issues or questions - Coordinating care between providers and following up on member cases - Providing regular check-ins and updates to members - Maintaining detailed notes and documentation in our member engagement platform
