Virtual PRN Primary Care Advanced Practitioner (NP/PA)
The Nurse Practitioner or Physician Assistant/Associate serves as an additional patient care provider virtually, augmenting and extending patient services provided by the health care team. The NP/PA will work closely with members of the team and have a reasonable amount of autonomy in care and medical decision-making for patients.
Job Requirements
- Board Certification (NCCPA or AANP or ANCC)
- Education: DNP, FNP, ENP, or PA from an accredited program
- Licensed NP or PA with current AZ licensure (Multistate licensure/Compact licensure preferred in AZ, MA, TN)
- DEA certification required or eligible
- Per diem role (flexible hours as needed, shifts could include days, nights, weekends and holidays)
- Bilingual in Spanish preferred
- Experience providing primary care, urgent care, emergency medicine or internal medicine for at least two years. Experience in virtual care/telemedicine preferred
- Knowledge of computer-based data management programs and information systems, as well as medical records and EMR/EHR technology
- Excellent verbal and written communication skills
- Internet access per Revere Medical policy
Responsibilities
- Evaluate, diagnose, prescribe, treat, and educate patients of all ages by phone or telemedicine (this includes on call after-hours support for current market clinic providers, high-risk patient population calls direct to provider support, escalations for Care Coordination team support for our high-risk patient population during business hours (including urgent TCMs), and call center triage support during business hours for VBC patients)
- Must be able to evaluate/treat/document in chart 3-4 patients per hour
- Schedule follow-up appointments with patients as needed
- Provide inbox support for providers on PTO/extended leave
- Provide refill inbox support for providers
- Order, administer, and interpret diagnostic labs and studies
- Recording patient medical histories in the electronic medical record
- Create patient care plans and implement existing care plans
- Participate in team meetings and conferences to enhance an interdisciplinary approach to health care delivery
- Maintain current level of knowledge relative to professional practice
- Maintain all required professional documentation, such as licenses, certifications, DEA as well as Revere Medical educational requirements
- Collaborate with physicians MD/DO on the Care Team as needed
- Reports to APP Director Virtual Care
