ROLE OVERVIEW
The Care Coordinator — Registered Nurse is a pivotal field-facing role at PRVNT. You will serve as the primary point of human connection for PRVNT members, translating our AI-driven health insights into real-world clinical action. This is a mobile, patient-first role requiring deep nursing competence, exceptional interpersonal skills, and the ability to work autonomously across hospital visits, diagnostic labs, home-care settings, and medical tourism support.
You will be the trusted face of PRVNT in a patient’s most vulnerable moments — whether accompanying them to a specialist appointment, administering an IV drip at home, or coordinating a seamless hospital admission for an international health tourism guest.
KEY RESPONSIBILITIES
1. Patient Escort & Hospital Coordination
- Accompany PRVNT members to scheduled hospital visits, specialist consultations, and procedural appointments across Chennai.
- Liaise with hospital reception, nursing stations, and treating physicians to ensure smooth check-in, prioritised queuing, and timely consultations.
- Maintain real-time updates to the PRVNT platform on patient status during visits.
- Arrange and manage transport for patients requiring assistance to and from medical facilities.
2. Diagnostic & Laboratory Trip Management
- Escort patients to partner diagnostic labs and radiology centres for PRVNT-ordered biomarker panels (180+ parameters) and wearable-triggered follow-up tests.
- Coordinate specimen collection, phlebotomy (where licensed), and ensure proper chain of custody for samples.
- Track and relay lab results to the PRVNT clinical team and update patient health records on the platform.
- Educate patients on pre-test preparation requirements (fasting, hydration protocols, etc.).
3. In-Home Clinical Services
- Administer intravenous (IV) fluids, vitamin infusions, and prescribed IV medications at the patient’s home in accordance with PRVNT’s clinical protocols and physician orders.
- Conduct routine vital signs monitoring including blood pressure, blood glucose, SpO2, temperature, and weight at home.
- Perform wound dressing, catheter care, and other nursing procedures as prescribed.
- Dispense and counsel patients on medications, dosages, and schedules as directed by PRVNT clinicians.
- Operate and troubleshoot wearable monitoring devices (ECG patches, continuous glucose monitors, fitness trackers) and upload data to the PRVNT platform.
4. Stay-In / Extended Home Care
- Provide extended or overnight in-home nursing attendance for post-discharge recovery, elderly care, or complex case management, as assigned.
- Ensure patient safety, comfort, and adherence to prescribed care plans during extended home stays.
- Communicate proactively with the supervising clinician about any changes in patient condition.
5. Medical Tourism Patient Support
- Act as a dedicated care concierge for PRVNT’s international medical tourism patients visiting Chennai for specialised treatments, surgeries, or wellness programmes.
- Coordinate airport transfers, hotel-to-hospital logistics, and appointment scheduling.
- Provide cultural sensitivity and language support (or assist with translation services).
- Accompany patients to all clinical touchpoints throughout their India visit.
- Facilitate smooth communication between international patients and local treating teams.
- Assist with documentation, medical visa queries, insurance pre-authorisation, and discharge summaries.
- Remain on-call during the patient’s stay to address urgent health needs or escalation.
6. PRVNT Platform & Documentation
- Maintain accurate, real-time electronic health records (EHR) on the PRVNT platform for all patient interactions.
- Log vitals, procedures, medications administered, and patient-reported outcomes after every visit.
- Flag abnormal readings or clinical alerts to the supervising clinician immediately.
- Participate in care plan reviews and contribute clinical observations to AI-generated health insights.
7. Preventive Health Education
- Educate and motivate patients on PRVNT’s prevention-first philosophy: lifestyle modifications, nutrition, physical activity, stress management, and sleep hygiene.
- Support patient onboarding onto the PRVNT app — guiding them through profile set-up, biomarker dashboards, and wearable integration.
- Conduct telephonic or in-person follow-ups as part of PRVNT’s chronic care management protocols.
QUALIFICATIONS & ELIGIBILITY
Essential
- Bachelor of Science in Nursing (B.Sc. Nursing) or GNM (General Nursing & Midwifery) from a recognised institution.
- Valid registration with the Tamil Nadu Nurses and Midwives Council (TNNMC) — certificate mandatory.
- Minimum 2 years of post-qualification clinical nursing experience in a hospital, clinic, or home-care setting.
- Demonstrated competency in IV cannulation, IV infusion administration, and phlebotomy.
- Working proficiency in Tamil and English (spoken and written); knowledge of Hindi or additional languages is an advantage.
- Willingness to travel within Chennai and suburbs for patient visits.
- Smartphone proficiency — comfortable using health apps, messaging platforms, and digital records.
Preferred / Advantageous
- Experience in home healthcare, concierge nursing, or patient care coordination.
- Exposure to medical tourism, international patient services, or premium hospital concierge teams.
- Certification in Basic Life Support (BLS) / Advanced Cardiac Life Support (ACLS).
- Knowledge of wearable health technology and remote patient monitoring tools.
- Post-basic specialisation in Critical Care, Oncology, or Diabetology is a strong plus.

WORKING CONDITIONS
- Field-based role: primarily home visits, hospital and lab accompaniment, and on-site patient support across Chennai.
- Rotational shifts including mornings, evenings, and occasional overnight/extended stays as required.
- Weekend and public holiday availability is expected (compensated per PRVNT’s leave and pay policy).
- PRVNT will provide a dedicated work mobile device, the PRVNT Nurse Care App, and all clinical consumables (IV sets, dressing kits, gloves, etc.).
- Candidates must possess a valid two-wheeler driving licence or be willing to use PRVNT-arranged transport/app-based cabs for field visits.
