<strong>Role Overview<br></strong><br>
</div><div>The <strong>Resident Care Coordinator</strong> provides <strong>case management, care coordination, and behavioral support</strong> for adults with developmental and intellectual disabilities. This role focuses on <strong>resident transitions, retention, and holistic care planning</strong>, partnering closely with admissions, caregiving staff, and health & wellness teams.<br><br>

<strong>Key Responsibilities<br></strong><br>
</div><ul>
<li>Coordinate <strong>person‑centered care plans (CQI)</strong>
</li>
<li>Provide <strong>case management</strong> and behavioral/emotional support</li>
<li>Support caregivers through training and conflict resolution</li>
<li>Assist with <strong>admissions, transitions, and discharge planning</strong>
</li>
<li>Collaborate on life‑skills classes, enrichment programs, and activities</li>
<li>Maintain communication with families and interdisciplinary teams</li>

<strong>Qualifications<br></strong><br>
</div><ul>
<li>
<strong>Bachelor’s degree in social work (BSW)</strong>, <strong>Special Education</strong>, or related field</li>
<li>
<strong>3-5 years’ experience</strong> supporting adults with developmental disabilities preferred</li>
<li>Background in <strong>case management, behavioral health, or mental health services</strong>
</li>
<li>Strong leadership, counseling, and organizational skills</li>
<li>Commitment to ethical care and resident advocacy</li>
<li>Christian faith alignment preferred</li>

<strong>Requirements<br></strong><br>

<strong>Safety‑sensitive position.</strong> Must meet state and organizational clearance requirements, including: fingerprint clearance, background check, drug screening, CPR/First Aid (or ability to obtain), and APS registry verification.<br><br>
