</ul><div>Key Responsibilities<br><br>
</div><div>• Perform evaluations of ADL/IADL function and home safety<br>• Develop individualized treatment plans<br>• Provide therapy to improve independence and safety<br>• Educate patients and caregivers<br>• Recommend adaptive equipment and home modifications<br>• Collaborate with interdisciplinary care team<br>• Maintain timely and accurate documentation<br><br>
</div><div>Qualifications<br><br>
</div><div>• Active California OT license<br>• Home Health or geriatric experience preferred, not required<br>• Strong communication and clinical skills<br>• Valid driver’s license and reliable transportation<br><br>
