Health Services Director (RN)- Intermediate Care Facility

What You’ll Do

• Lead and supervise a team of LPNs, including performance management and development
• Oversee all clinical services, including medication administration, treatments, and care coordination
• Provide direct patient care through assessments, evaluations, and clinical interventions
• Collaborate with interdisciplinary teams to develop and implement individualized care plans
• Monitor resident health status and ensure accurate, compliant medical documentation
• Support residents with developmental disabilities, dementia, and complex medical needs
• Educate residents and families on health conditions and care plans
• Assist with chronic disease management and preventive care initiatives
• Perform specialized procedures (DME fitting, specimen collection, CPAP management, etc.)

Requirements

• Active Florida RN license (required)
• BSN required; MSN preferred
• Minimum 6 years of nursing experience
• Minimum 3 years with developmentally disabled populations
• Supervisory or nurse leadership experience strongly preferred
• Valid Florida Driver’s License
• Ability to pass Level II background check, drug screening, and TB test

Summary

PHP/FPDC is an intermediate care facility for the intellectually and developmentally disabled. We care for individuals 24/7 in a set of four (4) homes that hold up to 16 individuals.
