Summary: The Intake Specialist is responsible for providing diagnostic assessments and supportive services for a diverse population in a fast-paced Mental Health Rehabilitative Core Service Agency. The clinician will facilitate intake assessments that includes adults, adolescents, or a combination of both. They are deeply familiar with other services and resources within the community and work closely with them to provide information and support when required.
Primary Job Responsibilities:
- Perform comprehensive assessments and refer patients for further care and services within the agency and/or the community
- Assess patients for risk of suicidal and/or homicidal thoughts or behaviors and complete safety plans as needed
- Utilize GAINS-SS & DLA-20, and PHQ-9 assessment tool to obtain needed information for each consumer
- Coordinate/engage clients to develop a person-centered treatment plan for services
- Prepare discharge summaries
- Complete all documentation within 48 hours
- Make referrals and collaborate with referral sources regarding client progress
- Attend all scheduled programmatic meetings (e.g., clinical supervision, staff meetings, etc.)
- All other clinical tasks as assigned
Job Skills:
- Detail-Oriented, highly organized, flexible
- Strong oral and written communication in English and strong interpersonal skills
- Technologically competent
- Familiarity with EMR systems andwilling to be trainedon applicable systems
Education/Experience:
- Master’s Degree in a human service or related field
- Licensed in the District of Columbia in one of the following licenses (LGPC, LGSW, LPC, LCSW, LMFT, Psychologist, Psychiatrist, APRN and/or RN)
- Two years minimum of direct-care clinical services with individuals with mental health diagnoses
