Role Summary:
The Patient Coordinator serves as the central liaison between patients (both OPD and IPD), the founding doctor, and internal clinical teams. This role ensures seamless patient navigation, from initial consultation through treatment and post-care follow-up, by maintaining patient records, recording case history, and maintaining compassionate patient communication.
Key Responsibilities:
- Coordinate day-to-day patient management including appointment scheduling, admissions, and follow-up planning
- Maintain and update patient records, case history, treatment timelines, and progress notes with accuracy and confidentiality.
- Liaise with clinical teams (nursing, diagnostics, pharmacy, and specialist consultants) to relay case history and track patient progress across departments
- Act as the primary point of contact between patients/families and the doctor for scheduling, reports, and care-related queries
- Track pending investigations, reports, and follow-up actions, ensuring timely closure and flagging delays to the treating physician
- Prepare case summaries and status updates ahead of consultations, rounds, and tumor board or case discussions
- Ensure patient documentation aligns with institutional protocols and applicable regulatory/compliance requirements (e.g., clinical documentation standards, patient confidentiality norms)
- Coordinate admission, discharge, and transfer processes for IPD patients in collaboration with nursing and administrative staff
- Escalate clinical or logistical issues affecting patient care in a timely manner
Criteria:
- Graduate degree (Life Sciences, Hospital Administration, or related field); healthcare/hospital background preferred
- 1–2 years of experience in patient coordination, case management, or front-office clinical operations, preferably in oncology or a multi-specialty hospital setting
- Working knowledge of patient record-keeping systems and case documentation practices
- Familiarity with clinical workflows in IPD/OPD settings and coordination across multidisciplinary teams
Location & Reporting:
- Based in Thane
- 3 days in a week at our Khar hospital
- Onsite job, 6 working days every week
- Reports to the Founding Doctor
