Job Summary:
The Rounding Care Coordinator supports patients during hospital stays and post-discharge by conducting non-clinical assessments, coordinating care, assisting with discharge planning tasks, closing care gaps, and ensuring smooth communication between patients, providers, and the care team.
Role and Responsibilities
Position Competencies:
1. Visit patients at bedside for non-clinical coordination
2. Gather information on discharge needs and barriers
3. Document patient-reported symptoms and needs in EMR
4. Assist with discharge planning tasks as directed
5. Conduct outreach to support patient care plans
6. Assist with medication reconciliation (verification only)
7. Close care gaps (AWVs, screenings, immunizations) and more...
Qualifications/Position Requirements:
1. Education: High School Diploma or equivalent. Completion of MA or CNA program required
2. Experience: - 1+ years' experience in a medical office, hospital support role, or care coordination
3. Certification/Licensure: MA or CNA certification, MA diploma or RMA required; Valid FL Driver License required
4. Training and Knowledge Required at Time of Hire: CNA or MA program curriculum completion
