Employment Type: Full-Time 
Schedule: Monday–Friday, with occasional evening/weekends. Hybrid, in-office time is required! Fully remote applicants will not be accepted.  

 

The Strategic Partnership Specialist is responsible for identifying, developing, and maintaining relationships with key partners to support organizational growth and strategic initiatives. This role serves as a liaison between the organization and external stakeholders, ensuring alignment of goals, strong communication, and mutually beneficial outcomes. This position contributes to strategic outreach and market development efforts, ensuring alignment with regulatory requirements and organizational capacity. Works closely with referral partners, leadership, clinical teams, and intake staff. This role helps ensure referrals are effectively converted into clinically appropriate, high-quality, and compliant admissions. Additionally, this role supports the growth of all PIHC service lines and assists in planning and executing community healthcare education initiatives and outreach events to engage providers and the broader community.

 

Responsibilities

Strategic Growth Planning

  • Collaborates on annual growth strategies for agency service lines
  • Partners on the analysis of market trends, referral patterns to identify expansion opportunities
  • Supports new program development, service line expansion, and geographic growth planning
  • Partners with leadership to set census goals, referral targets, and growth KPIs
  • Identifies opportunities for diversification (payers, referral sources, specialty programs)

Referral Development & Relationship Management

  • Builds and maintains relationships with hospitals, physicians, discharge planners, SNFs, assisted living, and community partners
  • Conducts regular in-person and virtual outreach visits to referral sources
  • Serves as liaison for strategic accounts and high-volume referral partners
  • Develops referral retention strategies and addresses barriers to admission conversions
  • Represents the agency at healthcare networking events and community meetings

Data-Driven Performance Management

  • Co-manages the tracking and reporting of referral activity, conversion rates, and census trends
  • Analyzes intake outcomes to identify lost referrals and process improvement opportunities
  • Monitors payer mix and referral profitability to guide growth strategies

Uses CRM or EMR data to evaluate outreach effectiveness Marketing & Program Development

  • Collaborates with marketing team to develop outreach materials and campaigns
  • Assists in event planning and launching of specialty programs (palliative care pathways, disease-specific programs, hospital partnerships, etc.)
  • Identifies opportunities for brand visibility within the healthcare community
  • Ensures messaging accurately reflects clinical capabilities and regulatory scope

Operational Alignment

  • Works with Intake, Clinical, and Scheduling teams to align growth efforts with staffing capacity
  • Helps prioritize referrals that fit agency expertise and compliance standards
  • Participates in admission review discussions for complex or high-risk referrals
  • Supports onboarding of new referral partners and education on agency services

Compliance & Ethical Growth Practices

  • Ensures all outreach activities comply with Medicare marketing regulations and anti-kickback statutes
  • Maintains ethical referral practices consistent with healthcare compliance standards
  • Educates referral partners on eligibility requirements to promote appropriate referrals

Requirements

Qualifications

Required

  • Bachelor’s degree in Healthcare Administration, Business, Marketing, Nursing, or related field (or equivalent experience)
  • Demonstrated success in referral development and census growth
  • Valid driver’s license and ability to travel locally for outreach

Preferred 

  • Experience in both Home Health and Hospice service lines
  • Familiarity with CRM systems and healthcare data reporting tools
  • Strong understanding of Home Health and Hospice eligibility criteria and workflows
  • Minimum 1-3 years experience in Home Health, Hospice, or post-acute healthcare growth/marketing
