Partners In Home Care is seeking a caring and grounded Chaplain to provide spiritual and emotional support to patients, families, and caregivers across our Home Hospice Program, Hospice Inpatient Center, and Palliative Care Program.

Job Title: Chaplain
Department: Hospice and Palliative Care
Employment Type: Part-Time
Schedule: Schedule varies. Typically Monday–Friday, with participation in on-call rotation one week at a time, every 4 weeks.
Job Summary
As a key member of the Interdisciplinary Group (IDG), the Chaplain contributes to the plan of care and helps address the emotional and spiritual needs that arise during serious illness and end-of-life.
This role includes crisis support, bereavement involvement, and participation in an on-call rotation to ensure patients and families have access to timely spiritual care 24/7.
Key Responsibilities
- Complete spiritual assessments for patients and families within required time frames
- Provide compassionate spiritual counseling, prayer, rituals, or supportive presence in homes, facilities, and the inpatient center
- Document spiritual assessments and interventions in accordance with agency policy and Medicare/ACHC standards
- Actively participate in IDG meetings and contribute to individualized care plans
- Participate in the on-call rotation to provide urgent after-hours support
- Offer crisis intervention and pastoral presence at the time of death
- Provide bereavement support in collaboration with the Social Work and Bereavement teams, including memorial activities and grief groups
- Educate staff, patients, and families on spiritual concerns, coping, and cultural considerations at end-of-life
- Build connections with local clergy and community spiritual leaders
- Support the Palliative Care Program through consultative spiritual care and team collaboration
Requirements
Education & Credentials
- Master’s degree in Divinity, Theology, Spiritual Care, or related field preferred
- Clinical Pastoral Education (CPE) preferred
- Board Certification (BCC/ACC) strongly desired
- Ordination, endorsement, or recognized standing with a faith community
Experience
- At least one year of spiritual care experience in a healthcare or hospice setting preferred
Skills & Abilities
- Compassionate communication and counseling skills
- Ability to support individuals of diverse faiths, cultures, and belief systems
- Strong documentation and organizational skills
- Comfort providing on-call and crisis support
- Valid driver’s license and reliable transportation
