Job Summary
The Occupational Therapist (OT) will assess, reassess, and plan care for participants in accordance to policy, procedures, and regulations. The OT provides and recommends therapeutic exercises, provides teaching, and retraining about activities of daily living (ADLs) to promote independence and safety. The OT delegates to and supervises care provided by the OT Assistant and Rehabilitation Technician. The OT also recommends modification in participant’s home to enhance safety and independence in ADL.

Specifications
Education and Special Training: Graduate of an OT curriculum accredited by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American OT Association.
Experience: Two years experience as an OT is preferred. At minimum one year of experience working with frail and/or elderly populations is required.
Licensure, Registry or Certification Required: Licensed OT in North Carolina (NC); Valid NC driver’s license and vehicle.
Knowledge, Skills, and Abilities: Demonstrates clinical competency with geriatric population; Acts within the scope of his or her authority to practice; Knowledge of and adherence to local, state, and federal compliance regulations; Meet a standardized set of competencies for the specific position description established by PACE of Guilford and Rockingham Counties and approved by Centers for Medicare and Medicaid before working independently.
With or without reasonable accommodations: Ability to bend and or stoop frequently; Have independent mobility in order to climb stairs and walk distances; Lift and position patients of all weights and heights utilizing correct body mechanics; Lift and carry pieces of durable medical equipment; Lift and transport at least 50 pounds during the course of providing care, treatment and/ or services; Utilize correct body mechanics to transfer, lift, and position participants; Read, write, and speak in English; Communicate effectively both orally and in writing; Ability to clearly see medical records and various documents; Ability to perform repetitive hand/wrist movements.
Summary
Key Responsibilities
· Conducts initial assessments and reassessments of participant’s condition and functional status. Communicates participant changes to team members.
· Develops treatment program, sets goals with participant and caregivers, and provides treatments.
· Documents interventions and outcomes in medical record according to agency policy and procedure.
· Coordinates 24 hour care delivery.
· Provides training to participants and family members, friends, and PACE of Guilford and Rockingham Counties’ staff members.
· Participates as a member of the Interdisciplinary Team (IDT). Contributes in care planning. Communicates changes in participant to other team members.
· Attends staff meetings.
· Other duties as assigned.
Environmental Factors
Task with potential for exposure to blood borne pathogens include, but are not limited to the following:
1. Teaching and assisting with participant ADL such as mouth care, toileting, bathing, shaving, etc.
2. Teaching and assisting with ADL with participant with non-intact skin
Demonstrates knowledge of and supports PACE mission, vision, values, standards, policies and procedures, operating instructions, confidentiality standards, and the code of ethical behavior.
