Job Summary
Under the general supervision of the Clinical Director, the Center Manager for PACE of the Triad has leadership and oversight responsibilities for members of the PACE Interdisciplinary Team (IDT) functions to assure that plans of care are developed and operationalized in compliance with all federal and state regulations and guidelines. This position is responsible for ensuring the delivery of high-quality care and services to participants and support to their caregivers. This position is responsible for oversight of 24-hour care coordination.
Requirements
§ Education/Experience and Special Training: Bachelor’s degree in healthcare administration, public health, business administration, or a related field, or Master of Health Care Administration preferred. At least three (3) years of supervisory experience in healthcare setting are required. Experience in PACE, Home Health or Long-Term Care preferred. One year experience working with a frail or elderly population strongly preferred. In the absence of such knowledge, receive appropriate training from the PACE of the Triad on working with a frail or elderly population upon hiring. Experience working in Electronic Health Records (EHRs) highly preferred.
Before working independently, meet a standardized set of competencies for the Center Manager position established by PACE of Triad. Before engaging in participant care, be medically cleared for communicable diseases and have up-to-date immunizations and vaccines. Have not been convicted of criminal offenses related to involvement in Medicaid, Medicare, other health insurance or health care programs, or social services programs under title XX of the Act. Have not been convicted of criminal offenses pertaining to physical, sexual, drug, or alcohol abuse. Have not been excluded from participating in Medicare and Medicaid programs.
Licensure, Registry or Certification Required:
§ Active license or certification in a health care field preferred. Up to date registry if applicable. Basic Life Support Certification from the American Red Cross preferred.
Knowledge, Skills, and Abilities:
Knowledge of the complex and unique needs of older adults. Able to foster collaborative working relationships and possess strong leadership, management, and supervisory skills. Applies critical thinking skills to effectively solve problems. Must have strong interpersonal, group facilitation and consensus-building skills. Able to manage multiple projects through effective time and priority management. Demonstrates strong oral and written communication skills. Able to learn, teach and research laws, regulations, and best practices. Knowledge of cost containment systems and quality improvement.

Communicate effectively both orally and in writing. Ability to clearly see as evidenced by a vision screening and adequately review medical records and various documents. Communicate via telephone. Home visits to participants and sites of network providers required.
Summary
Management Responsibilities:
§ Responsible for supervising assigned IDT staff as indicated in the PACE of the Triad’s organization chart. Supervisory responsibilities may include, but may not be limited to, nursing staff, physical, occupational and speech therapists, social worker staff, home care coordinators, recreational therapist staff, dietary staff, and primary care as it relates to administrative functions as part of the responsibilities of the IDT.
§ Directs and oversees the hiring, training, and evaluations of all assigned members of the IDT. Coordinates staff development and assists with other human resource issues in conjunction with the PACE of the Triad HR department.
§ Responsible for coordinating all IDT activities, including participant care planning, the Service Determination Requests (SDRs) process, the Appeals procedure, the Grievance procedure, daily morning meetings, IDT meetings, and coordinating and leading participant/caregiver conferences.
§ Responsible for timely coordination of all PACE voluntary and involuntary disenrollment activity.
§ Works collaboratively with PACE of the Triad’s medical director, other clinical leaders and subject matter experts to assure effective clinical practice guidelines and best practices are in place to ensure the best care is being provided to participants by the interdisciplinary team.
§ Serves as a member of PACE of the Triad’s 1st Team.
§ Responsible for PACE program compliance with all current federal and state regulations governing PACE and for participating in, preparing for, and assisting with all federal and state audit activity.
§ Participates in the development and implementation of PACE of the Triad’s Quality Improvement (QI) plan.
§ Responsible for establishing and maintaining effective relationships with leadership staff, contracted network providers, and federal and state authorities.
§ Reviews IDT documentation routinely to ensure alignment with regulations, as well as PACE of the Triad’s policies and procedures.
§ Responsible for reviewing regulations and educating the IDT and appropriate staff accordingly.
§ Responsible for reviewing IDT policies and procedures and making recommendations as appropriate.
§ Ensures fiscal solvency and fiscal viability through efficient and effective IDT management.
Participant Care Responsibilities:
§ Responsible for coordinating 24-hour care delivery to PACE of the Triad participants.
§ Responsible for overseeing PACE participants quality of care, safety and well-being.
§ Facilitates morning meetings and other IDT meetings as required.
§ Responsible for ensuring participants and caregivers are satisfied with services provided through PACE of the Triad.
§ Responsible for ensuring participant rights are upheld and works collaboratively with caregivers.
§ Responsible for ensuring an efficient and compliant enrollment process for newly enrolled participants is upheld in concert with the Intake staff.
§ Takes proactive steps to prevent dissatisfaction by participants and caregivers, as well as voluntary disenrollments.
§ Partners with SNF leadership to endorse quality care of for participants in their care.
