Position Summary
The Social Services Coordinator plays a vital role in supporting the emotional, psychosocial, and transitional needs of residents within our skilled nursing and short-stay rehabilitation community. This individual serves as an advocate for residents and families, ensuring a positive experience from admission through discharge, while collaborating closely with the interdisciplinary team to promote high-quality outcomes and resident satisfaction.
Key Responsibilities
- Conduct comprehensive psychosocial assessments upon admission and throughout the resident’s stay
- Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team
- Provide counseling, emotional support, and crisis intervention to residents and families
- Assist with discharge planning to ensure smooth, safe transitions to home or other levels of care
- Coordinate with community resources and external agencies to support resident needs
- Facilitate care conferences and communicate updates to residents and families
- Address resident concerns, grievances, and quality-of-life needs in a timely and professional manner
- Support advance care planning discussions as appropriate
- Maintain accurate, timely documentation in accordance with state and federal regulations
- Promote resident rights, dignity, and person-centered care
Qualifications
- Bachelor’s degree in Social Work, Human Services, or related field required (BSW preferred)
- Experience in skilled nursing, rehabilitation, or healthcare setting preferred
- Strong knowledge of discharge planning processes and community resources
- Excellent interpersonal, communication, and conflict resolution skills
- Ability to manage multiple priorities in a fast-paced healthcare environment
- Knowledge of state and federal long-term care regulations
