A Case Facilitator I - Child and Adolescent operates under supervision of the assigned Area Director or designated QBHP/LP. A member of a multidisciplinary treatment team, primary job duties include providing case management and activities of daily living skills to child and adolescent consumers.
o Links, monitors, and refers clients to appropriate community resources
o Coordinates care with appropriate internal and external parties
o Ensures appropriate behavioral health services are provided
Education: High School diploma or equivalent.
Experience: Must meet OBHP requirements. Two years’ experience in mental health or community services preferred.
Licenses/Certification: Valid state issued drivers license.
Shift: first shift, full time, 40 hours weekly, Monday-Friday (Note: two days roll into second shift hours up to 7pm.), no weekends or holidays.
Salary is based on experience.
