RN/ LPN- Community Appointment Nurse

Position Overview:
We are seeking a qualified and passionate RN/LPN Appointment Nurse to join our team. The ideal candidate will play a crucial role in coordinating and facilitating client appointments, conducting medication reviews, and ensuring that our clients receive the highest standard of care.
Key Responsibilities:
Client Appointments:
- Transport clients to medical appointments and community engagements.
- Communicate effectively with healthcare providers regarding client needs and concerns.
Client Advocacy:
- Advocate for clients’ needs and preferences in healthcare settings.
- Maintain open lines of communication with clients, families, and the healthcare team.
Documentation:
- Accurately document client interactions, assessments, and care plans in compliance with regulatory requirements.
- Maintain confidentiality of client information.
Qualifications:
- Unencumbered RN or LPN licensure in Iowa.
- Experience in community-based nursing or HCBS preferred.
- Strong knowledge of medication management and nursing practices.
- Excellent organizational and communication skills.
- Ability to work both independently and as part of a collaborative team.
- Valid driver’s license with a good driving record and the ability to obtain a chauffeur’s
