ESSENTIAL DUTIES AND RESPONSIBILITIES (include the following but are not limited to):
- Experience working with adults with I/DD preferred, or experience treating individuals with physical, cognitive, and psychological ailments
- Timely completion of daily notes, progress notes, and discharge summaries
- Coordinating with physical therapist to adapt and/or modify the course of treat as indicated
- Communicate with supports coordinator to promote implementation of recommendations in home and community settings
- Educate staff on recommendations to be performed outside of therapy sessions
- Adheres to DDD guidelines for habilitative therapy services
- Follows directive from supervising Physical Therapist and Clinical Director
- Maintain accurate records of daily notes, progress, and treatment plans, ensuring compliance with legal and ethical standards
- Work with healthcare providers to ensure effective client care and participate in multidisciplinary team meetings as needed
QUALIFICATIONS REQUIRED:
- Degree from a Physical Therapy Assistant program from an accredited institution
- Valid NJ state licensure as a Physical Therapy Assistant
- Minimum of 2 years of clinical experience; preferably with individuals with developmental disabilities (special needs)
- Strong understanding of the unique needs and challenges faced by individuals with intellectual and developmental disabilities
- Excellent communication, leadership, and organizational skills
- Ability to develop and implement strategic plans and initiatives
- Experience with Foothold AWARDS preferred
