Active AZBBHE licensure (LMSW, LAC, LAMFT, or LAAC) required; LCSW, LPC, LMFT, or LIAC preferred; 3+ years substance abuse experience preferred; valid Arizona driver's license; Class I Fingerprint Clearance Card; BLS certification; TB test; immunization record; substance abuse testing.
