POSITION SUMMARY:
Under the supervision of the Clinical Supervisor, the Intake Coordinator provides comprehensive coordination for the referral, assessment, and admission of clients entering NAC programs. This includes eligibility verification, review of referrals, requesting additional needed information, , submission of prior authorization, and assist with admission into programming. Occasional weekend and or evening work will be required.
RESPONSIBILITIES:
· Provide excellent customer service to clients and referring agencies
· Receive and acknowledge referrals (fax, email, phone, walk-ins) within 2 business hours
· Creates client file in Electronic Health Record (EHR); ensuring accuracy and completeness of of record
· Screen referrals for necessary clinical information
· Reach to client or referral source out for missing referral information
· Obtain referral documents
· Schedule behavioral health assessment upon receipt of referral
· Set admission appointment and ensure all appointments are logged in EHR
· Prioritize admissions for priority populations (Native American, pregnant, IV substance use, etc.)
· Prepare admission packets, maintains stock of packets
· Ensure all required forms are completed and signed
· Complete full admission process including searches and belongings check
· Escort clients to residential as needed
· Coordinate detox and MAT services throughout referral, assessment, and admission process
· Coordinate transportation and pick-up for PWC/PMP, including sending email request
· Make referrals to internal and external programs based on assessment
· Enter and manage Unite Us referrals
· Utilize the Olio portal for referral and coordination activities
· Conduct referral and admission audits as assigned by supervisor
· Maintain strong referral relationships with contractors, collaborators, and community agencies
· Attend departmental or agency meetings as required

EDUCATIONAL / WORK EXPERIENCE REQUIREMENT:
· Bachelor’s degree in behavioral health field with 2 years’ experience in substance use or mental health treatment
· OR Associates of Arts Degree in behavioral health field and 4 years’ experience (Behavioral Health Technician)

· MS Office proficiency
· Strong interpersonal skills with an ability to work effectively with a wide range of people, teams, managers, supervisors, and vendors.
· Demonstrate excellent written and oral communication skills.
· Excellent customer service skills.
· Experience in working with the Native American population preferred.
· Must demonstrate critical thinking, problem- solving and organizational and time management skills.
· Proficiency in electronic health record systems
· Must be able to work well with others in a team approach.
· Possess and maintain a valid Arizona driver’s license
· Fingerprint Certificates within 90 days of hire.
· Bilingual- Spanish desired
· Understand the dynamics of substance use and demonstrate the ability to integrate the knowledge for practical application.

· Know the economic, educational, and social problems of Native Americans and referral sources available
