Clinical Support and Data Collection
- Conducts structured in-home visits to collect clinical and functional information to assist in determining clinical eligibility and nursing home level of care for prospective PACE participants, under the direction and supervision of a Registered Nurse.
- Gathers medical history, medication lists, functional status information, psychosocial data, home safety observations, and caregiver support details.
- Accurately document collected information in the electronic medical records system.
- Communicates findings to the supervising RN and interdisciplinary team for review and determination of eligibility.
Community Outreach & Relationship Development
- Builds and maintains referral relationships with hospitals, skilled nursing facilities, assisted living facilities, physician offices, and community organizations.
- Educates providers and community partners on PACE program benefits and eligibility requirements.
- Represent Palm Beach PACE at community events, health fairs, and professional networking functions.
Enrollment Support
- Partners with the enrollment team to ensure a smooth transition from assessment to enrollment.
- Provides factual clinical information during enrollment meetings under RN supervision.
- Escalates clinical questions requiring assessment or determination to the supervising RN.
Documentation & Reporting
- Maintains accurate, timely documentation of assessments and outreach activities.
- Submits weekly activity reports in Salesforce and census updates to the Director of Business Development.
- Monitors assessment outcomes, referral productivity, and conversion metrics.
Interdisciplinary Collaboration
- Communicates collected data and observations to physicians, Registered Nurses, social workers, therapists, and other interdisciplinary team members.
- Participates in enrollment discussions as requested, providing factual clinical observations within the LPN scope of practice.
- Supports implementation of care plans as directed by the supervising RN.
- Performs all other duties as assigned.
SUPERVISORY RESPONSIBILITIES
This position does not have supervisory responsibilities.
QUALIFICATIONS EDUCATION and/or EXPERIENCE
To perform this job successfully, an individual must be able to perform each primary function satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
- Graduate of an approved practical nursing program and a minimum of one (1) year of LPN experience required; experience in geriatrics, community health, or home care preferred.
- Must travel between all PACE centers and participants’ homes.
- Possesses strong clinical judgment, communication, and organizational skills, with the ability to multitask, adapt to change, and collaborate effectively with interdisciplinary teams and caregivers.
- Exhibits cultural sensitivity, inclusivity, and respect while working with diverse populations across varying socioeconomic and cultural backgrounds.
- Self-motivated with strong initiative and problem-solving skills.
LANGUAGE SKILLS
Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the public. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of the organization.
MATHEMATICAL SKILLS
Ability to work with mathematical concepts and to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
REASONING ABILITY
Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
CERTIFICATES, LICENSES, REGISTRATIONS
- Active LPN License in the State of Florida
- Active CPR certification
- Valid driver's License in the State of Florida
