About the role
701B Level of Care Assessor is responsible for completing and managing annual level of care recertifications for the Palm Beach PACE program.
PRIMARY FUNCTIONS
- Completes assigned LOC 701B Assessments monthly
- Completes all paperwork and documentation needed for the 701B assessments/upload into PACE logic
- Faxes or emails completed assessments to the CARES Office in DOEA
- Enters the completed data on the 701B Spreadsheet Folder located on the R shared Drive
- Reports to Regional Director of Enrollment, any delays or issues in completing the 701B assessments in a timely manner.
- Ability to complete a thorough assessment/evaluation of a participant and their home environment
- Follows instructions from an equipment’s instructional manual.
- Reads maps, street finders and to follow written and oral directions.
- Performs all other duties as assigned by manager.
QUALIFICATIONS (EDUCATION and/or EXPERIENCE)
- 16hrs DOEA certification training, preferred
- Experience in Cognify – Pacelogic, preferred
- Have access to a car as this job requires a face to face visit in order to complete the assessment, so the staff member will need to go to the participant’s home as needed
