Position Summary:
Responsible for assessing patients and families who request admission to Montgomery Hospice. During assigned visits, the Admissions RN provides a comprehensive assessment of patients to be admitted and initiates the plan of care and functions in other nursing capacities as needed. Must be willing to work in both Montgomery and Prince George's counties.
Duties and Responsibilities:
Accurately assesses patients and family needs.
Identifies and recommends medical equipment and supplies that will assist in maintaining patient’s comfort and caregivers’ ability to provide care.
Identifies the need for additional services, including nutritional assessment, physical therapy, occupational therapy and speech therapy and initiates appropriate referrals.
Assesses need for and educates caregivers on safety and emergency preparedness, including safe use of medical equipment.
Conveys physician’s requests to appropriate staff on appropriate forms.
Minimum Qualifications:
Current state of Maryland Nursing License. CHPN preferred.
Graduation from an accredited nursing program. B.S.N preferred.
Two years of recent clinical experience, preferably with hospice and or critical care.
Previous hospice or home experience strongly preferred.
