We operate Florida PACE (Program of All-Inclusive Care for the Elderly) Centers, the largest PACE program in Florida, which serves as health plan, healthcare provider, and social center designed for older adults with complex medical needs. Job Title: RN PACE

 

Job Summary

Under the general supervision of the clinic manager RN and medical director of FLORIDA PACE CENTERS, provides nursing care to all persons enrolled in the program.

 

Essential Job Functions

·    Performs comprehensive nursing history and physical on new referrals.

·    Creates appropriate care plan and participates in IDT.

·    Evaluates and triages participants with episodic illnesses

·    Refers participants to medical director or medical specialist as indicated

·    Actively participates in family conferences.

·    Participates in interdisciplinary team meetings, committees and work groups as assigned.

 

Job Requirements

Education

·    Graduate from an accredited school of nursing

 

Experience:

·    At least 1 year of RN or 3 years of LPN required

·    Minimum one-year experience working with a frail/elderly population

 

Licenses/Certifications:

·    Current RN license by the Florida State Board of Nursing

·    Valid Florida Driver’s License & Proof of Insurance

·    BLS certification required

·    ACLS preferred

 

Abilities Required

·    Bilingual English/Spanish, knowledge of Creole a plus

·    Proven clinical leadership

·    Ability to get along with others in an interdisciplinary team environment

 

Functional Demands

 

Environment Work Conditions

·    Normal working condition with adequate lighting and ventilation

·    Ability to assist transferring patients and ambulation which may involve lifting, pushing, and pulling of patients from 50-150 pounds

 

Infectious Material Exposure:

·    Routine exposure to blood/body fluids/tissues

·    Ensures that resident’s/patient’s rights are adhered to

·    Demonstrates professionalism and accountability

·    Demonstrates a caring attitude consistent with Miami Jewish EmpathicareSM toward MJH residents, patients, family members, employees, and other facility guests

·    Ensures confidentiality and security of patients’ medical information

·    Identifies and utilizes appropriate channels of communication

·    Able to speak, read and write English

·    Able to think and act calmly to meet unusual occurrences of the job

·    Adheres to the organization’s Mission, Vision and Values

·    Participates in departmental activities, meetings and in-services and follows established guidelines

·    Maintains a safe working environment
