<strong>Position Type<br></strong><br>

<li> Full-Time </li>
<li> Salaried Position </li>
<li><strong>One full week(7 days) of on-call coverage required and included in salary</strong></li>
</ul><div>
<strong>Responsibilities<br></strong><br>
</div><ul>
<li> Provide skilled nursing care to hospice patients in homes, facilities, assisted living communities, and other care settings in accordance with the plan of care </li>
<li> Perform routine visits, admissions, recertifications, PRN visits, and death visits as needed </li>
<li> Assess patient needs and develop individualized care plans in collaboration with the interdisciplinary team </li>
<li> Administer medications and treatments as ordered while ensuring patient comfort and safety </li>
<li> Monitor changes in condition and communicate updates to physicians and team members promptly </li>
<li> Educate patients and families regarding hospice services, symptom management, coping strategies, and end-of-life care options </li>
<li> Support the emotional and spiritual needs of patients and families during this critical time </li>
<li> Complete all clinical documentation accurately and <strong>within 24 hours of each visit</strong>
</li>
<li> Participate in <strong>biweekly IDG (Interdisciplinary Group) meetings</strong>
</li>
<li> Maintain compliance with Medicare, state regulations, and company policies </li>
<li> Provide after-hours support during assigned on-call rotation </li>
</ul><div>
<strong>Qualifications<br></strong><br>
</div><ul>
<li> Current and active Texas Registered Nurse license in good standing </li>
<li> Minimum <strong>2 years of nursing experience required</strong>
</li>
<li> Hospice, home health, palliative care, or end-of-life care experience preferred </li>
<li> Strong clinical assessment, time management, and critical thinking skills </li>
<li> Excellent communication and interpersonal skills </li>
<li> Ability to work independently while collaborating effectively with a team </li>
<li> Compassionate, patient-centered approach to care </li>
<li> Current BLS/CPR certification required </li>
<li> Reliable transportation, valid driver’s license, and current auto insurance required </li>
<li> Experience with EMR systems preferred (Axxess experience a plus) </li>
